As I specialise in portraits, I tend to use an acid free off white paper usually 130 gsm. All my work is printed using the Giclee printing process which doesn't like pure white paper. (doesn't pick up the tones very well)

Usually an off-white medium - as in not too fine but not too coarse. I don't like papers that have no 'teeth'

a paper with some tooth if I'm drawing with pencil, charcoal or conte, but a smooth paper if I am drawing with ink or marker.

A good old, cheap sketch pad. It also depends what you are drawing with: charcoal, pastel, pencil?
