In the sign-up pages, the title I'm choosing for my Hubpage
is 'I Love Book TV - when I enter the Url that seems to be
suggested in small letters under the line...
http://hubpages.com/I_Love_Book_TV, I get a message that
slashforward is not allowed but no suggestions as to another
What is the correct way to format this URL?

I think the problem is you pasted in the the http://hubpages.com/hub/ part of the URL into the box. This will be added automatically, so just enter the part of the URL that comes after that, for instance:
By the way, if you just enter a title, a valid URL should be created for you automatically based on your title (it will remove or replace characters that aren't allowed from your title).
