Ads by google appear in your hub because while publishing your hubs you do not un-check the option of "This hub may be commercial".
If you un-tick the option in the settings tab and chose 'none' in the ads section. No ads will appear on your hub.
Coming back to your question.
To be approved by google adsense, first you hve to register and apply. This will contact you via email on whether or not your account has been created. They will provide you with an id, which when you add to the 'Affliate Settings' in 'My account" section, once approved by hubpages.
Then your google adsense will be set up.
If you are a new user who has not applied for adsense, ads will still come on your hubs till you stop it by above method.
No. The ads will show up whether you have an AdSense account or not. If you do have an account, 60% of the ad impressions are yours. If not, they are all HubPages'.
