I have published over 40 hubs and I click the Start a New Hub to do it. But this time that I did I got something different than I usually get. This does not have sub-categoriies. Also on the next page I do not get all those buttons like for Bold and Italicized.
I tried it again and got what I usually get. So do you know what I did differently to get this? Also what is this for? http://hubpages.com/hubtool/accessible/
You clicked on the simplified version of create a new hub. Back out and start all over. When you go back in to 'Create a new hub' right under those three words and highlighted in blue says, 'simplified version.' You just clicked the wrong button.
Wow - I'm glad I read this - I've never seen the simplified button! I would have been as stumped as the OP. Any idea what a person would use that version for? And why?

I use it to get feedback from readers of my troubleshooting hubs about whether the hub helped or not. If the poll showed results and a high percentage of "yes" votes, it might be an incentive for readers...
