Patient Enrolment and Consent to Release Personal Health Information

Recently, Ontario healthcare providers have been making their patients sign Patient Enrolment and Consent to Release Personal Health Information forms. The Patient Enrolment part of the form is a loyalty pledge. IE. I won't see any other doctor but you unless it is an emergency. The Consent to Release part ensures that all your medical information ends up in one place (the Ministry of Health and Long-Term Care) where it can be easily accessed if/when necessary instead of lost in the nether or scattered among various medical locations that service you.

Whether you've moved to a new community or are just unhappy with the level of service your current doctor is providing, you need to de-enroll from this program before you can find a new doctor. You can either contact your current healthcare provider and ask to be removed or, if like me you're so ticked with your current doctor you don't even want to deal with them anymore, you can remove yourself with a simple phone call to Service Ontario ... Well, I say, "simple" but anyone who has ever tried calling Service Ontario knows it isn't simple.

Fortunately, I was able to get in touch with someone who provided me with a map through the maze of options the automated service offers. It saved me hours of of frustration and waiting, so I have decided to pay this kindness forward to you.

Keep Your Information Up-to-Date

Please note, your health card information must be up to date to complete this process. This means the Ministry of Health and Long-Term Care must have your current address and contact information.

To De-Enroll Yourself ...

Have your health card handy and call 1-888-218-9929;

The first option you will be given is to pick your language. Press 1;

Once you've gone through, there is no need to wait for the auto-operator to finish speaking. Simply press in the following in order as each new connection is made:

This will bring you directly to a live person who can help you. You will need to provide your health card number, current address and other personal information (for verification purposes).


What's Next?

According to a helpful commenter, it takes approximately 48 hours for your cancellation to show up in the system. After that, you cancontact Healthcare Connect at 1-800-445-1822 or visit their website to register with your local patient waiting list.

You can actually call health care connect after 48 hours. That's how long it takes for the cancellation to show up in the system.
