Shock is a state where there is inadequate circulating blood volume and inadequate tissue perfusion of the vital organs.

Shock is mainly classified into

  • Hypovolemic shock

  • Cardiogenic shock

  • Septic Shock

  • Obstructive shock

  • Anaphylactic shock

Hypovolemic shock is the most common type. It results in inadequate circulating blood volume

Cardiogenic shock is caused when the heart fails to pump effectively

Septic shock is caused by vasodilatation in severe infection such as infection by gram negative bacteria

Anaphylactic shock is caused by a severe allergic reaction to an allergen

Obstructive shock is caused when there is an obstruction to the flow of blood as in cardiac tamponade which may result in circulatory arrest

Full thickness Burns can cause considerable fluid loss and result in shock

Here are a few multiple choice questions on shock which will help you to revise the subject

Failure of the heart to pump effectively causes the following type of shock

  • Anaphylactic

  • Cardiogenic

  • Hypovolemic

  • Septic

------------------------------------

Answer: Cardiogenic

Overwhelming infection and resulting vasodilation can lead to the following type of shock

  • Anaphylactic

  • Cardiogenic

  • Hypovolemic

  • Septic

-----------------------------------

Answer: Septic

Trauma to the spinal cord and resultant loss of autonomic and motor reflexes below the injury level can lead to the following type of shock

  • Cardiogenic

  • Hypovolemic

  • Neurogenic

  • Obstructive

--------------------------------

Answer: Neurogenic

Important points to remember in Shock

  • Head low position increases the filling pressure of right ventricle and in turn increases the cardiac output

  • Cardiogenic shock is an emergency and it requires immediate attention before the vital organs are damaged from ischaemia

  • Early revascularization in patients with Myocardial Infarction helps to prevent revascularization.

Cardiac tamponade can result in the following type of shock

  • Cardiogenic

  • Hypovolemic

  • Neurogenic

  • Obstructive

-----------------------------------

Answer: Obstructive

The skin is warm and dry in the following type of shock

  • Cardiogenic

  • Hypovolemic

  • Neurogenic

  • Obstructive

--------------------------------

Answer: Neurogenic

What should be the position given to a patient in shock?

  • Head low

  • Fowler’s

  • Semi fowler’s

  • Supine

----------------------------------

Answer: Head low

Dopamine is an ionotrope used to increase blood pressure in patient with shock

The acid base balance which commonly accompanies shock is

  • Metabolic acidosis

  • Metabolic alkalosis

  • Respiratory acidosis

  • Respiratory alkalosis

-----------------------------------------------

Answer: Metabolic Acidosis

Which stage of shock is associated with development of metabolic acidosis?

  • Initial

  • Compensatory

  • Progressive

  • Refractory

-----------------------------------

Answer: Progressive

Which stage of shock initiates the compensatory mechanisms to maintain the BP and blood flow to the vital organs?

  • Initial

  • Compensatory

  • Progressive

  • Refractory

-------------------------------------

Answer: Initial

Patients with large partial-thickness or full thickness burns can develop

  • Anaphylactic shock

  • Cardiogenic Shock

  • Hypovolumic shock

  • Septic Shock

----------------------------------------

Answer: Hypovolumic shock

Vasodilation and resulting hypotension and increased capillary permeability is characteristic of

  • Anaphylactic shock

  • Cardiogenic Shock

  • Hypovolumic shock

  • Septic Shock

--------------------------------------------

Answer: Septic shock

Intravenous fluids can be lifesaving in a patient with Hypovolemic Shock

  • Septic shock presents with high cardiac output and hypovolemic shock presents with low cardiac output

  • Since a patient in shock has decreased circulating blood volume, it can damage the vital organs like heart, kidneys and brain

I knew of someone who almost went into shock, so the topic was very interesting for me. I like your approach of beginning with a clear and easily understandable introduction about shock and its categories, and then proceeding with questions, as it involves readership participation. This approach would be great for aspiring nurses:).

Hi Au fait,

Thanks for the visit and comment.

Don't worry if you got some of the answers wrong. If you have not studied about these things previously it is difficult to answer them

I knew a little bit about shock before, but not much. Your explanations were helpful and I did OK on the first questions because you had given explanations about the things being tested. When I got to the things where there was no explanation I didn't do so well, but then I had never heard of these things before.

Really enjoyed learning about shock. Voted up and will share.

Hi kristyleann

I am glad that you got most of it correct. This knowledge will not be waste even if you are not working in health care right now.
