Kala's Poem

In the Hidden Camp, to the northern section past Haedrig, there is a wooden overlook where a noblewoman named Kala stands. During the quest Blood and Sand, go to her and start the conversation Noble Dreams with her. After the conversation is through Kala's Poem journal will be automatically added to your inventory.

During the quest Blood and Sand, after rescuing Covetous Shen, head back to the Hidden Camp and go through all the available conversations you have with him. Eventually, you will come to one labeled A Case of Mistaken Identity, which once completed, will yield you this journal.

Baal, the Lord of Destruction

The next spot which allows us to pick up the eight and final journal in the Cain series of Act II is in the Vault of the Assassin while searching for Kulle's blood beneath the Desolate sands. This happens during the quest Blood and Sand, but like always, you can farm journals from any of the spawn points for Cain's Satchel, the best being south of Khasim Outpost waypoint and right by the entrance to the Oasis, from the Path to the Oasis waypoint. It is entirely possible to find 2 of Cain's Satchels in this dungeon, which helps if you missed one prior.

Kulle's Journal, Part 1

During the quest Blood and Sand, while you are searching for the blood of Zoltun Kulle, you will eventually have to enter the Vault of the Assassin to retrieve some of it. At the end of the dungeon, right by where you pick up his blood, look around and you should find a satchel labeled Zoltun Kulle's Journal. Open this pack up to make the first of his four part journal fall out.

Kulle's Journal, Part 2

You will find this next journal (given you've went to the dungeons in the order I did) at the entrance to the Cave of the Betrayer, where the next one of Zulle's packs lays in wait. Note that it is somewhat likely that you can go to any of Kulle's packs in the Desolate Sands to get the 1-3 journals, although this is unconfirmed. (comments appreciated)

Kulle's Journal, Part 3

You will find the third pack right next to the blood of Kulle's found at the second level of the Cave of the Betrayer, also right next to the portal which will take you back to the Desolate Sands. In this installment, he goes on about how the Black Soulstone is the greatest gift to mankind ever. Yeah, let's see how that turns out.

Crumbling Journal

This journal is located somewhere randomly within the entire exterior of the Desolate Sands area in Act II. This one can be a real pain to find, because it is found off a corpse labeled Skeletal Remains, which looks just like all the Forsaken Prisoner skeleton corpses you can search throughout the area. The one main difference which you can tell them apart by, is the Skeletal Remains has no shackles or armor on it, just being a bare skeleton.

The Ancient Portal Device - Lyndo's Journal

In the Desolate Sands the is a large tile which can sometimes spawn known as the Ancient Portal Device. This is a semi-event which potentially gives you one of 10 different possibilities. You will need to find this for the journal which sits next to it however, in a satchel by a corpse labeled Garot's Corpse. There are two spots which can spawn big tile events in this area, northeast and southwest of the gap which always sits in the middle of the region. Take the time to check where exactly the spots are (you'll know by what spawns there) and run the sands until you come across the device.

Note: It took me nearly 30 runs through the Desolate Sands before I ever saw this event. Hopefully the experience is not as excruciating for you, however be forewarned it has the potential to be.

Kulle's Journal, Part 4

At the end of the quest The Black Soulstone in Act II of Diablo 3, you will fight Zoltun Kulle himself as the second boss i the act. Upon defeating him in the Soulstone Chamber, the final part of Kulle's journals will fall from his body. There is nowhere else to get this journal, but its pretty hard to miss because of this.

Leah's Journal, Part 12

Check Leah's belongings in the right corner of the hidden camp one more time at the end of Act II, during the quest The Scouring of Caldeum to find the last in her series of journals found in this act.

Blackened Journal

During the quest the Scouring of Caldeum, while you are rescuing the civilians in the Caldeum Bazaar, look around for a Charred Iron Wolf Corpse. It should be in the bottom left section which is opened after saving (or letting them die) some refugees in the larger courtyard. Check the corpse for the Blackened Journal to fall out.
