THE BLUE EU HEALTH CARD

Questions you should ask yourself BEFORE you seek prior approval:

Have you within the last 2 months up to departure (6 months of travel over a month)

1. Been hospitalized?
2. Been outside the general medical supervision?
3. Been prescribed a change in your medicine? (For example, the second dosage, termination or commencement medicine)
4. Or have you written up for referral to or on waiting lists for assessment / treatment?

Can you answer yes to any of the above questions, you should seek prior approval. If your existing or chronic illness has not led to answer yes to any of the above questions, you need not seek prior approval.

When you apply for prior approval, you will receive a written response, which may be:

1. An unconditional approval
2. A conditional approval, subject (except for coverage for certain conditions)
3. A refusal.

This means that a distinction between a full recovery without reservation, a cover subject for your chronic condition, or that we can not meet you. In some cases we need additional information and you will receive a letter from us on this.

COMMON CONDITIONS
