Indemnity Dental Insurance Plan: An indemnity dental insurance plan allows you to select your own dentist. This plan is also considered as "fee-for-service". Fee for Service means that you pay a fixed amount of for dental visit, but you have an annual limit on coverage for dental spending, and specific coverage limits may apply to individual dental procedures.

Self-Insurance Dental Plan: Self-insurance dental plan is more or less very similar to in nature to indemnity plans, but you are allowed to select your own dentist.

Closed Panel Dental Plan: Closed panel dental plan offers limited facilities and the number of dentists available to you. But this plan is comparatively cheaper.

Capitation Dental Plan: Capitation dental plans designate specific dentists for dental treatment. The insurance companies have contracts with these dentists and in return they are paid a fee regardless of whether dental treatment was required or not.

Preferred Provider Organization (PPO): PPO dental plans are limited to a group of dentists who are available to provide dental care at a reduced cost. This is one cheap dental insurance plan.

Dental Care Service Plan: Dental care service plans use a group of dentists who form a non-profit organization to provide dental services at fixed fees.

Direct Reimbursement Plan: Direct reimbursement dental plans allow employers to directly reimburse employees for dental services they receive.

Choosing the Best Dental Insurance Plan

Before choosing a dental insurance plan you need to study and understand what is covered by dental insurance. Let us say, you need a dental filling with a composite material. Many insurance companies (Or Plans) may reimburse composite filling costs at a rate of an amalgam filling and you may have to pay the balance amount out of your own pocket.

Again, there are other dental treatments such as cosmetic treatments or multiple treatments that are not covered completely by most dental insurance plans. Many of them have limitations and other terms and conditions. You need to understand these limitations and terms and conditions.


Usual, Customary, and Reasonable (UCR) Fees

Usual, Customary and Reasonable or UCR is the most common term used by dental insurance companies on their Explanation of Benefits (EOB) statement to identify the fee for dental treatment. This is determined by insurance providers based on the typical costs associated with various dental procedures.

For example, if your procedure costs $100, your dental insurance provider may have a UCR for the procedure of $70. Here you may have to pay the balance $30 out of your own pocket.

It is therefore advised to ask the dentist's finance manager about dental treatment costs prior receiving the treatment. The dental finance manager can call your insurance provider to pre-determine the out-of-pocket costs associated with your insurance plan.
