Sham is a student studying in class 2. Life for sham is a living hell. His teachers are very disappointed with his progress and perennial complain that he is inattentive in the class or is of to sleep very often. His peers do not involve him in any of their games as he has a tendency to freeze from time to time. his parents are worried that their child has no interest in life and is really a problem child. He was diagnosed to have ADHD and seizure disorder and put on heavy medications to no avail. At this juncture, he was referred to a psychiatrist. Initially his parents were reluctant to go to a psychiatrist, suggesting that their son was not mad, but later on the insistence of their family doctor met a psychiatrist. The doctor suggested them to a test. Subsequently, he was diagnosed to have narcolepsy.With proper therapy and behavioral training he has shown significant improvement in his day to day affairs.
Narcolepsy is a sleep disorder wherein the patient has excessive daytime sleepiness.It often presents during adolescence and nearly 1/4Thof the cases have positive family history.The sleep attacks may be very brief (microsleeps)resulting in attention deficits and mood disturbances.Other features being cataplexy -- Sudden onset of loss of muscle tone partial/complete in response to a emotional stimulus. Hypnagogic (sleep onset),hypnopompic(on waking)hallucinations and sleep paralysis. PSG and multiple sleep latency test is the gold standard for the diagnosis of narcolepsy. Treatment generally involves combinations of medications to combat daytime sleepiness and REM sleep suppressants to prevent cataplexy.
