Personally, I would look at things she enjoys, and try and set her up with a special day. I like to get my wife a gift that she can hold and touch....and also a special spa day. Usually it starts with a pedicure, then a manicure then a massage, a facial and then get her hair done. Some spa's have a price menu to where you choose a certain number of items, then when she arrives, she picks what she wants.

If she is not the type for a spa day, then find something else that she likes and arrange it.

Does she enjoy art...then maybe a museum day is in order.

Or perhaps does she want a day to go shopping by herself. You can stay home with the kids, and make it happen for her.

Perhaps, she may even just want a day the two of you can spend together. Take off work if you can arrange it, and make her feel pampered and loved.

If you can afford it, prior to her birthday, go to a decent card shop, and take your time selecting a greeting card that fits your relationship.

Once you get the card, write something personal in it that she will find meaningful.

If you can't buy a card, then you can make one. Even if it looks like a first grade art project, it is still something personal from you to her.

If she has a favorite restaurant, take her there. Some restaurants have special celebration meals. Call and ask if they have specials when you are looking to make reservations. And on that note, if the restaurant accepts reservations, by all means, make a reservation. Nothing is worse then taking her to the restaurant to find out there is a 1-2 hour wait.

If you can't afford that, then perhaps you can cook her favorite meal for her at home.

My wife used to say, you can't go wrong with jewelry. Now she has so much, she has changed her mind about that, but for many women that still holds.

Flowers are also a good option. My wife tells me there is a waste of money because they don't last long, but I know she really enjoys their beauty while they are in bloom.
