As far I can remember Rajakumari was the first known item girl. In those time, glamour or oomph was not existing. But at the time of prevailing standards, she was considered the most sexist item girl.
Suddenly Jayamalini appeared on the silver screen dazzling in Tamil and Telugu films. She was voluptuous and very sexy dazing the fans all over south India. Her sister Jyhothilakshmi too dazzled and captured the fantasies of the audience. Another voluptuous and sexy Anuradha entered into foray as well.
These were the times when heroines of south India had to portray ice maidens and they are not suppose to show skin or even move around male members. Heroines were not suppose to portray image of glamour and sex. To counter this obstacle, they hit up the idea of bring one glamour character - thus giving birth to item girls and item songs. Since the, it become a norm to have on item song in every movie released.

Disco Shanthi, Kyuili and Silk Smitha too forward the legacy of the famous trio Jyothilakshmi, Jayamalini, Anuradha. In fact they took a step ahead in glamour and item numbers. They were features in big films and even the big directors felt the need to include their item songs in their movies
Now the glamour quotients have changed. Separate items girls are now seldom required as heroines themselves are doing the spicy roles in the movies. Roja, Meena, Rambha, Nagma, more recently Namitha, Trisha, Malavika, Asin, and Shriya. Some of them even appear in one item numbers.
