I am trying to get claim forms and have not yet recieved them send them to Marta T. Cuevas at 1014 Hanover St. Daly City, CA. 94014. For I too was taken advantage of by Verizons. And why is the wolf watching the fold? why aren't we dealing with the lawers and not the company( crooks) that took our money?
Again I called the 1-888-309-3847 number and left my name and address to get claim forms and why don't they have an option to fax the paper work to us and there is no way the claim forms were ever going to be sent to us. I got my forms, however still have not heard anything from these people or seen any cash return.It has been 3 1/2 months now I think we should have seen some type of action by now!!!!!!

recieved the card back in August went online submited claim, called 888 number in January was told in progress, now late March 888 number is just a recording what a scam
I went online to the site to print the necessary forms along with my requested information to mail off when verizon sent me the postcard on how and what to do to file for your claim. This was back Oct 2008. It is now July 10, 2009 and still havent heard a thing!!!! Anyone actually file and get an answer or is everyone in the dark???
You go to the site they are evaluating the claims still...it could take up to two years before we receive or hear anything sorry for the bad news to those of you who are in need of your money

I have already sent my claim alongwith necessary documentation on 9/13/08 and have received by them on 9/15/08. As of today I have not received any information about status of this claim. Any one let me know the status of this proceedings.
