I agree with others that there were many factors: anorexia, drug abuse, stress. I think in the end, the prescription drug cocktail in his system suppressed his respiratory drive. Once he was in respiratory arrest, it was only a matter of time before his heart stopped.

It would seem so, yes. He did. Considering (what is reported) is emanciated state, anorexia, his lack of eating and fluid intake habits reported by his staff; and his addiction to pain medication. He died this way. He was self destructing. So, I can agree, that he died of heart failure.
