but he is doing good in sexual intercourse the only thing is when penis get erection we can see blood in his small nerves, it may effects

Should not affect his erections but it would be best to see a doctor and ask him. In time the scratches will heal but he needs to have it looked at. Try searching webmd.com for Penis injuries and they might have some good info on it.
