I believe the underlying reason for why men and women don't understand one another is their refusal to accept (there are gender differences when it comes to mindset and behavior). Instead of accepting this as fact we hold onto our expectations that the opposite sex will eventually "see the light" and start to think the way we do or at the very least think from (our point of view) when approaching us.
Those men and women who are well read or have taken an interest in having better communication with the opposite sex tend to do better in this area. The vast majority of people however insist their gender is "right" and the problem is with the other side. It's the role of the person communicating to make sure he or she is understood. In order to accomplish that you have to understand not only how the "other half" thinks but how this (individual) thinks. The problem is we want to (simplify) things and say there is only ONE way to approach something, (Our way).
