Interesting question. Speaking just of the US, obesity wins hands down. The total medical costs of obesity totaled a whopping $147 billion in 2009.
Meanwhile, alcohol abuse and alcoholism cost only $26 billion in 2006.
These numbers do not include many of the indirect or non-medical costs, such as lost productivity.
There were 8 million alcoholics in the US in 2006, versus 72 million obese. The majority of Americans are overweight or obese.
