I have never seen anything like it; why do you think it is she is always called Kate Middleton; her maiden name still; while expecting their second child? It is always Kate Middleton. Why not Duchess Kate if they don't know her name. (Which I readily admit I do not but surely the British media would know it?)

Yes, that's probably it. I am sure they know her name, but more know her by Middleton.
It just seems so disrespectful to me. I have heard her simply called Middleton a couple of times. I am sure I wouldn't like it and it just surprises me that every year passes and still she cannot be called by her husband's name.

That's the tabloids for you. They saw her as a sort of 'crusader' or 'innovaror', just as Princess Michael was called 'Princess Pushy' by them because of her attitude problem. But then they both had 'attitude problems'..
