How to potty train a child with constipation "PTSD?"

My daughter has had severe constipation in the past, fissures, the whole works, and is still on a prescription laxative to control it. Our gastroenterologist has said that "PTSD" from her previous constipation may make it difficult to potty train her, and our experience seems to confirm it. She knows how to pee on the potty, but if she's anywhere near needing to have a bowel movement she'll hold it on the potty and pee and poop in her diaper no matter what we do. Is there a way to combat this, or will we just have to be content with delayed potty training?
