DEFLECTED NASAL SEPTUM. (D.N.S)
WHAT IS DEFLECTED NASAL SEPTUM ? ( D .N . S )
Deflected Nasal Septum ( D.N.S ) is a deviation of nasal septum to one side and it is very common. A D.N.S may or may not give rise to any symptom.
WHAT ARE THE CAUSES OF D.N.S ?
3) Pressure from one side.
4) Intrauterine Position of Head.
WHAT ARE THE TYPES OF D.N.S ?
The deviation may be Anterior,Middle,High or Low.
WHAT ARE THE SHAPES OF D.N.S ?
Deviation may be L,C or S Shaped.
WHAT ARE THE EFFECTS OF D.N.S ?
1) ON EXTERNAL NOSE;-
D.N.S may cause nasal deformity
2) ON NASAL SEPTUM:-
At the site of maximum deflection, there may be projection of bone which is called "SPUR". It may be so pointed so that it touches the lateral wall of the nasal cavity giving rise to Neuralgic
pain. The thin mucous membrane get ulcerated and there may be Bleeding.
D.N.S may cause Thickening of Nasal mucosa.
3) ON NASAL CAVITY :-
On the side of Deflection :-
The nasal cavity becomes narrow,turbinates are shrunken,aeration of the Sinuses does not
take place which give rise to SINUSITIS, and a creation of a negative pressure causes
On the opposite side of the Deflection :-
Nasal cavity is Roomy,Turbinates undergoes compensatory Hypertrophy, therefore a deflected
Nasal Septum to one side give rise to BILATERAL NASAL OBSTRUCTION.
ON Adjoining Tissue :-
Infection of Eustachian tube gives rise to Otitis Media.
Nasal Obstruction gives rise to Mouth Breathing which gives rise toDryness of Mouth and
Nasal Obstruction gives rise to improper sleep, leading to Lack of Concentration, Poor memory,
Headache and Mental Backwardness.
Various LARYNGEAL, PHARYNGEAL and TRACHEAL INFECTIONS.
WHAT ARE THE SYMPTOMS OF D.N.S ?
D.N.S. may be ASYMPTOMATIC OR SYMPTOMATIC.
SYMPTOMS may be due to Nasal Obstruction which are :-
1) Symptoms due to obstruction in air passage :-
a) Inability to Breath.
b) Bleeding from ulcerated mucosa.
c) External Nasal Deformity.
d) Change of Voice.
e) Pressure Headache.
f) Decrease sense of Smell ( Partial Insomnia ).
2) Symptoms due to Obstruction of Nasal Secreation :-
b) Otitis Media.
e) General Symptoms of Mental Retardness, such as Lack of Concentration,Poor Memroy
and Fatigue etc.
WHAT ARE THE SIGNS OF D.N.S ?
1) Obvious External Nasal Deformaty,whole Nasal complex is on one side.
2) There may be SPUR.
3) Nasal Cavity is Narrow on the Defected side and Roomy on the other side so compensatory
Hypertrophy of the Turbinate on the Opposite side.
4) Air Way on the affected side may be reduced or blocked.
WHAT IS THE TREATMENT OF D.N.S. ?
If D.N.S. is Asymptomatic then the Treatment is not needed.
If Symptoms are present then the Treatment is SURGICAL OPERATION which is called SUBMUCOUS OR SUBMUCOPERICHONDRIAL RESECTION. ( S.M.R. ).
WHAT ARE THE INDICATIONS OF S.M.R. ?
1) D.N.S. Producing Symptoms.
2) S.M.R. for other reasons :-
a) Cosmatic Nasal Surgery.
b) As an approach to Pituitary gland.
Through Septums after removing bones and Cartilage approach to other organs is made for
instance Sphenoidal Air Sinus, Trans-Septal approach to the Pituitary gland to destroy or
WHAT ARE THE CONTRAINDICATIONS FOR S.M.R. ?
ABSOLUTE CONTRAINDICATIONS :-
1) Age of the Patient. Small Children and Old People should not be Operated.
2) Local Acute Infections. In Acute Sinusitis, Operation is not done.
RELATIVE CONTRAINDICATIONS :-
1) Menstruation and Pregnancy.
2) Hypertensive Patients.
3) Tuberculosis or Diabetes.
