Bilirubin is of two types: Direct and Indirect.
A high direct bilirubin is usually indicative of liver disease or bile duct disease. In this case , other liver tests are abnormal ( although not always).
While , a raise in indirect bilirubin is usually a result of red blood cell breakdown ( i.e. a blood disease). Other liver tests are generally normal.
If both direct and indirect bilirubin are raised, then a liver disease is most probable.
This is only generally speaking. To decide whether, raised bilirubin is caused by liver disease or not, this depends on the whole panel of  medical history, examination, blood tests, and  imaging.

Can liver 'dis-ease' be a result of s/p gallbladder removal and common duct blockage?

Common bile duct obstruction may result when the duct is traumatized during surgical removal of the gallbladder. This is uncommon and it can result in raised direct bilirubin and some other liver enzymes. But , although these liver tests.

Does he have his gallbladder? Status post gallbladder removal up to ten years can create other issues with the biliary tree.
