This is usually a question asked in a competence based interview situation. You need to develop something that relates to the job you are applying for. For instance, if you were applying for a manager's job, you might think of a situation where you knew a member of staff was going to be off on long term sick leave (e.g. they were going to have an operation on their knee) and you would have to cover for their work. You might have had several options, such as hiring another member of staff OR covering for their work using other staff, or letting the work pile up. You would have to think which would be best and whether your current staff could do the work (with training), whether you had the budget and whether the work COULD be left (or maybe other work could be left instead).
You might be a football coach, where you anticipated problems with a young footballer and took measures to prevent these.
You might think of your own health, for instance, you were putting on weight and so decided to take up exercise and diet in order to remain a healthy weight.
