Depends on how your league is. My league personally is really heavy on QB's, but most others are not. For those leagues that rely more on the running backs, i would say Arian Foster should be number one. He was the leading rusher in 2010 and recently reported that he played through a torn meniscus during the season. This is the same injury that has A-Rod on the DL right now, so you know that if he could produce so well with that kind of injury that he could be even more of a nightmare for opposing defenses when healthy. I passed him up last year and it has hauntd me ever since. I personally don't believe in repeats for break out players like that(*cough* Cedric Benson, Steve Slaton, Ryan Grant *cough cough*) but I am convinced he will satisfy fantasy owners everywhere this year.
