    There's no particular cause, just the way they are designed. All disks fragment their data because of an inherent inability to store related data in contiguous spaces. So, whenever you update an existing file from your disk, the updated portion of the file that doesn't fit in the existing space where the file was is stored somewhere else. Whenever you want to open said document, the disk has to seek through for all the fragments before opening the file.
