Miat if you are having problems for your working conditions, you can request in an application addressed to your superior. As I do not know about the organizational settings you are working in, so all i can give is some general suggestions, but they will definitely work to persuade them that your problem needs their attention.
Start with short introduction of yourself, your name, department, employee id and period of work in a sentence.
Describe your daily tasks that you perform and express desire to increase your productivity.
Tell employer the specific things are hindering you to achieve your productive goals.
Close with expressing gratitude for being employed and being part of team.
Ask for action at earliest.
Finally, be very concise in your sentences and don't make the application long. One sentence for each idea above will suffice.
