Not sure what you mean. You send DMCAs directly to the person, site, or host of the offending material.
If you go to your hub in author or edit mode and click on 'Looks like your article has been stolen', you'll find the template. Just copy it, complete your details, and email/submit to the person, site or host.
I never fill in my full address though, as it gets sent to the thief.

If it shows on HP as stolen, you can get the DMCA text by clicking on author view, then clicking on 'Your hub has been stolen' then on 'File a DMCA' (you can't do it there but the text will appear for you to copy). It will include the link to the stolen copy as well as the link to your original.
You can use a service like Who Hosts This to find the hosting company that hosts the website the plagiarist's article is on. Then when you've found that, Google <hosting company> + abuse to find contact details (e.g. an email address or form) for reporting the content. (Replace <hosting company> by the name of the hosting company). Reporting to Google or other search engine provider will only deindex the offending website/page and not remove it from search results unless it's a Google site like Blogger or YouTube.
