I would take away Grandpa's car keys once his driving skills were determined as unsafe. Unsafe to Grandpa or others.
Of course give Grandpa a chance. Perhaps he is going through a rough patch and will return to better driving later.
When he can no longer remember where he is going, or can't find his way back home. Talk to him about it, he may decide when it's time. I know a man that gave up driving at age 91, he no longer felt safe behind the wheel...admitted he wanted to stop before he had an accident.
