I can't find any reference to this in the TOU except that a user may not "Engage in behavior that is overly promotional, including excessive linking to any one site or domain"
Can we link exchange if the linked to site is commercial, relevant to the content of the hub and beneficial to both "linkees"?
Link exchanges are prohibited and can result in your account being banned and in some cases an AdSense ban as well.

Are simple 2-way reciprocal links also prohibited if the link is external to HubPages?

Well link exchange is not encouraged but you can find such people on Digital point forums, V7N forums and SEO chat forums.
