I wondered this myself, so I did a lot of research on the subject. I found that it was recommended that you start cleaning your childs gums before they get their first tooth, and continue to brush once their teeth come in. I did this with my son and so far so good.

I started with all my children as soon as they got their first tooth. But didn't use a brush, my dentist had suggested using wash cloth and rub their teeth with it. Started using toothbrush after they started eating solid food.

Let them use a small soft toothbrush - under strict supervision - as soon as they are able to hold it. If they don't have teeth yet, they can brushing their gums. I wouldn't use any kind of toothpaste, however, until they can understand the concept of spitting it out.
