2 places for me, actually - Bangkok and Phuket. If it has to be the first? Then, Phuket - more because of the beach and food there, without the crowd like Bangkok.
For me, first place is Bangkok http://hubpages.com/hub/Thailand-4-Days-in-Bangkok
Second would be Chiang Mai. http://hubpages.com/hub/Thailand-3-Days-in-Chiang-Mai
Both places are very different and offer different experiences...
@ telltale...if you like the beach, i recommend Krabi and Pang Nga where not far from Phuket. There are many nice islands but less crowd compare to Phuket.
@ lilian_sg...next time if you visit Chiang Mai, try Chiang Rai and Mae Hong Sorn...Not far from Chiang Mai but more fresh and less crowd.
When I'm in Thailand my first priority to visit is Bangkok. I loved the place.

What's the best way to travel between Bangkok and Phuket or Chiang Mai?Plane? Train? Bus? Or rent a car and drive? I'm thinking of taking the train from Bangkok but it looks like I still need to take a bus part of the way.

I have been living in Thailand now for more than 2 years and I really do enjoy it. Everything is much cheaper than my home countries of Aus/NZ and Thai people are very nice.I do however find it hard living here as a foreigner with poor Thai skills sometimes haha. What did you think of Thailand...
