I'm thinking consult as many on-line articles as you can and then schedule a meeting with your supervisor. You might also want to practice ahead of time with someone at home too. Perhaps open the conversation with something to the effect of that you feel that you have contributed in many ways recently. I would suggest having a lot of examples ready of how you have helped the company ready.
Then find a way to tactfully ease the conversation towards how you feel you should be compensated for all that effort and why you feel you deserve it. Perhaps be open to negotiation too. You could start with a higher figure (within reason) that you think they may not go for so that you have some wiggle room to negotiate down to what you really desire. Chances are you won't get what you really want so think compromise and realize that something more is better than less. Good luck!
