If a published hub is edited then does it require to republish it? to make it effective

No! just editing and hit "Done Editing" is enough. No need to republish it.

When one publishes a hub and then goes back and edits it, can there be some indication that the hub has been edited? This way people who have seen it before can come back and look at the new changes/additions.
