If you filled out their form, it's a legal document in which you make a legal declaration. And, as such, they have to act upon it. The date of publication is easily proven, both by the copyright notice on the article and on the notification by Hubpages that the work is copied. I would fill in the form weekly until it gets taken down.
If it's possible to add an image to the form, snip the text using the snipping tool in Windows, save it and send it to them. Here's a cut down version:
