    Yes, we do because I know they don't get a lot of reimbursement for gasoline.  I usually give the pizza guy 4 or 5 dollars.

    Yes .  Around a dollar or so depending on the cost of food I purchased.,

    You definitely have to tip well on the delivery guys. They typically get screwed otherwise. And if they offer you a survey, I try to fill those out because that's how they organize schedules at some places.

    I give 10% of the bill for tip for delivery or a little more, depending on how long it takes for the food to get delivered. If I order a pizza that supposed to come in 40mn and it takes an hour, then the tip is pretty small.

    Yes, I figure that it would be required or expected to tip when a delivery has been made.

    Yes, the ultimate service is to have your food delivered to your front door.  And, if it arrives quickly, neatly and politely it's only fair.
