experience acute pain (rated 8/10). Returned to dentist, had some bone shaved off to below the gumline and hope that gum will grow over the bone. Pain has escalated, cannot sleep, cannot eat, nauseated with pain. Little relief. how long will this continue, is it getting better? is this part of the 'getting better' process?]
I hope you are feeling better by the time you read this post. I am not unfamiliar with that of which you speak. I understand how you feel.

First, I am not a dentist. So, medically, dentally, legally I am not qualified to speak on this subject. That having been said, I know of similar experiences and one week with pain, some insomnia, not eating normally is not uncommon. However, escalating pain and nausea could indicate an infection. Now, definitely alert your dentist and go to your doctor or hospital emergency room if you feel that you should. Perhaps you should even if you feel you shouldn't.
A good dentist, if he or she has done a thorough intake and is familiar with your dental history, any phobias, resistance to pain and the condition of your immune system, will take appropriate measures to make sure your dental procedure and healing period is painless as possible.
Some dentists will treat highly phobic dental patients with anxiety reducing drugs several days before the procedure. Also, a short course of antibiotics could be added to help eliminate a possibility of infection after dental procedure including removal or surgery if required. A good dentist will also spend at least ten minutes practicing visualization techniques and relaxing imagery that the dental patient can use during the dental procedure to help reduce the mental agony and pain of the dental procedure. If a patient has a high tolerance to the typical novacaine injection for pain, a very good dentist will prescribe a powerful pain reliever, taken orally, an hour or so before the dental procedure begins. From the time the patient sit in the dental chair to the time he or she gets out of the chair eyes should remain closed.
Your age, quality of your immune system, current level of health can be related to your resistance to infection or longetivity of pain. Death by dental infection resulting in heart attack is not uncommon. For the money spent and to be on the safe side, I would definitely return to your dentist, see a doctor or go to a hospital emergency room to be on the safe side. Why chance sepsis?
If the pain has escalated, consider getting another opinion from another dentist. Good luck.
