Why do most relationships fail ?
Lack of communication?
To much self indulgent?

Lack of honest communication is one. Lack of commitment is another. If you don't agree to stay together forever, why work to solve problems and come through difficult times? That's why 80% of living together couples with kids split before the child is a teenager versus 25% of married parents.
Lack of communication and commitment are some reasons that contribute to failures in relationship, But there are some factors we often ignore, unrealistic expectations, robotic behavior, not sacrificing or enduring to make relationship work.
