What do I do if someone uses my insurance licence to set up appointments with out my permission.
There must be a state board that governs the use of licenses. So contact them and have your license number deactivated. Then have them assign you a new one. If this person continues to use your license then -- because that license is no longer valid -- the act of using your license constitutes fraud which is a criminal offense.

What does your insurance license have to do with appointment setting? How did your license get in the hands of someone else? something sounds fishy here.

I've seen my photos used occasionally, but turn a blind eye if they've included a link back to the article in their reference list, or otherwise suggest they include one.
