How long should people date before sending holiday cards together?
In other words, what is an appropriate amount of time a couple should be established before sending out holiday cards with both names together, without it seeming silly or forced? Once they start living together? Not until engaged or married?

As per my view, couple shall date for at least 10 months. I know it's bit long time but It is a slow and steady process to know each other very well. In today's fast moving world, everyone wants everything within 2 to 3 months.
The more time couple would spend together more opportunity to know each other very well. It would avoid things like break-ups and divorce.

Who sends holiday cards these days? I haven't for over ten years!
I would say if the couple isn't at least living together no one would expect you to send out cards signed together.

Most people date more people than they will become exclusive with and most people become exclusive with more people than they will live with, and most people live with more people than they'll marry.
If I had sent out holiday cards with the name of every girlfriend I "happened to be with at the time" it would have been a joke!
At the very least everyone you send a card with both names should have met and spent some quality time with you as (a couple).
