What does this shoe mean?
It would depend on what kind of relationship he had with the lady. If there was a romantic kind of relationship, where "lust" drove the relationship, then I would think that the ONE shoe being sent to him, was a message telling him he knows where to find the other shoe, should he want it.
I wouldn't have any other explanation for it, except to say it was an insult, which I'm not really sure what the message would translate into.
