i think its comes from the middle age, when a castle was attacked the first who were rescued were the Ladies. Everytime the castle was attacked and everyone ran into the castle cellar, the knights stoped them with the words: ladies first!
I think it is more of being 'chilvarous' of a gentleman, when it comes to anything involving both genders.
It was considered good manners (before Feminism) to raise your hat to a lady , open a door for a lady, pull back a chair for a lady or give up your chair to a lady and allow ladies to go first.
