Yes. I used to perform a lot. One time I had a really quick costume change and my friend didn't button the back of the dress all the way. Another time I was dancing a salsa. My partner accidentally swiped the dress off my shoulder, but recovered before anything got too revealing. I think I have a ton of these stories. But they all have to do with performances. Nothing in "real-life" but I almost wish it did happen in real life. Less people would see it.
Not a "malfunction" so much as a "wardrobe incident". I live in NYC and ride the crowded subway frequently. The weather becomes very hot and maneuvering the subway system becomes a bit cut throat, or cut pants i should say. I didnt realize that the back of my pants got sliced open when they got caught on something and unknowingly exited the subway with a buttcheek completely exposed. I didn't know it for blocks, until a doorman pointed it out to me. I died that day.
