Do you return phone calls when the caller doesn't leave a message?
On my work phone, I always answer it if I'm around and I always call back if someone leaves a voicemail asking me to do so. But when someone I don't know calls me and doesn't leave a message, I tend not to call them back.
So what does everyone else do? Call back no matter what, or just assume that if they couldn't be bothered to leave a message then it wasn't that important to begin with?
No, because it is a common tactic of spammers to call and not leave a message to test if it is a working phone number, and then they log when you call or answer on a later try as the best time to bug you.
If it is someone you know trying to get you, they'll either call back later or try to reach you through another channel.
