This is the Apostles Creed and is called the Apostles’ Creed because it is supposed to be a record of what the apostles taught. The Apostles' Creed is as follows:

I believe in God, the Father Almighty,
the Creator of heaven and earth,
and in Jesus Christ, His only Son, our Lord:
Who was conceived of the Holy Spirit,
    born of the Virgin Mary,
    suffered under Pontius Pilate,
was crucified, died, and was buried.
    He descended into hell.
The third day He arose again from the dead.
    He ascended into heaven
and sits at the right hand of God the Father Almighty,
whence He shall come to judge the living and the dead.
I believe in the Holy Spirit, the holy catholic church,
    the communion of saints,
    the forgiveness of sins,
    the resurrection of the body,
    and life everlasting.

there may have been something earlier but no written records - it wasn't written down til the first council of Nicaea in 325 CE and was then sent out as a sort of loyalty oath to test for orthodoxy.  within 60 years there were already significant differences that resulted in another church council, and kept the heresy-creation-suppression business going for several centuries

The Apostles' Creed is exactly as stated by Sharon Douglas.  It originated sometime in the 2nd century.  The Nicene Creed originated in the 4th century as something of an expansion of the Apostles' Creed.  Each grew out of the need for a uniform statement of what Christians believed, and in response to what they believed to be heretical teachings at the time - The Apostles' Creed came as a response to Gnosticism, the Nicene Creed as a response to Arianism.  The general principles of the two creeds are essentially identical.  The wording is slightly different.  The Nicene Creed explicity states that we (I) believe in ONE God, while the Apostles' Creed does not.  The Arianist believed that the Father, Son, and Holy Spirit were three separate gods (the Son and the Spirit having been created by God), rather than three persons of one God, and the Church fathers at the time felt that to believe that Jesus was a creation of God rather than God incarnate made Him subordinate, and took away from the truth of a Triune God.

And..don't forget the Athanasian Creed which is longer than both the Nicene and Apostles Creeds together.  We recite it in church on Trinity Sunday and it seem to take forever.
