Zero on average-- maybe once a month,usally if traveling.

Walk the dog about one mile a day, some gardening.
How often do you cook a meal for yourself? (other than just heating something up):
almost every day, sometimes twice a day.
How often do you eat out at a restaurant other than fast food?:
About once, sometimes twice a week, but it's usually breakfast or lunch.
Do you frequently eat fruits and vegetables?:
Yes. Grow a lot myself.
Do you consider yourself a healthy eater?:
Mostly.

Only when nothing else is available or convenient. Actually fast food places in my area are few and far between which is fine because the choices are terrible.

No, especially for the quality of food.

Two or three.

Usually two, and maybe a snack or two,

I could lose 10 lbs.. but I'm OK

Nothing, but I'm eating almost all healthy now.

How often do you participate in physical activity? (i.e. sports, gym, jogging): walk everyday; gardening; high energy person in general; ride my bicycle - long 35-40k trips every 2 weeks - should do more.
How often do you cook a meal for yourself? (other than just heating something up): once a day - actual cooking;
How often do you eat out at a restaurant other than fast food?: couple times a month;
Do you frequently eat fruits and vegetables?: every day
Do you consider yourself a healthy eater?: used to be a really, really healthy eater...getting back there....
What is your primary reason for eating fast food?: convenience - only in the a.m.

How many meals do you eat per day?: 3 x; plus 1 or 2 snacks; basically eat when I'm hungry.
Are you overweight or obese?: no; underweight and am trying to even it out right now.
What is the biggest obstacle stopping you from eating healthier?: me; life changes; will get it back on track; used to cook for others; now adjusting to just cooking for me; big change but I'll get it back to the way I used to put more effort into cooking, its preparation and nutritional value; been eating more carbs than usual just to put some weight back on.

Number 1 is caregiving for my husband and Number 2 is not much of an appetite

Often when the caregiving starts weighing on me to much I have prepared meals delivered to the home which are frozen and I simply have to heat. On the good days, I cook.
