I do my prayers usually following the Christian charismatic way using the ACTS acronym as guide as I pray from my heart.
Adoration, contrition, thanksgiving and supplication, although they may not be prayed in this order but I do start with Adoration where I praise and express my respect as I worship and honor God in Christ Jesus and invoke the Holy Spirit's presence on the prayer.
Then I thank him for all the blessings and the beautiful things I can relate to during the occasion or any event that is worthy of thanking him.
Then I may do the supplication asking for blessings and answers to my petitions or requests or ask for impromptu supplications from the rest of the group that we may pray for each other in our petitions, that in any eventuality, it is God's will not our will be done.  Lastly, I may do the contrition, asking forgiveness for my sins and for sins of the community or I can interchange this with supplication as I so see fitting the event or situation.  Sometimes I add 3 Glory be's as I acknowledge and proclaim the glorification of God as part of my ending prayer.

Usually, it depends upon one's beliefs and the person who is praying.

Depends on who you are praying to, I guess.  As a Christian, I look to the Bible for guidance.  It give examples on how to pray- such as the Lord's Prayer, or the example in Acts that Edwinoel gave.  I believe that God gave us these examples so that we wouldn't be so intimidated when talking with Him.  I think He knows that we all start out with that question of how to approach Him.

For me, I don't use a specific format.
I speak from my heart, because God knows our heart better than anyone.  No matter how you talk to Him, He always hears you.

No, there is no set or specific way to pray.  I believe praying from the bottom of your heart is most important.  It does not matter how you talk to God, just be honest with Him because He already knows the truth.  The Bible also says, "Pray without ceasing." I Thessalonians 5:17.  I believe this means that we should stay in regular contact with God.  I like to think I talk to Him as if He were next to me each day.

it depends on the person, its religion, culture, purpose and inside life...these are parameters that will set a person into religious prayer, or meditation...that can be cone so many different ways. I love the way meditation feels like going into a full closet, emptying it, then triage everything and store back what we want to stay in, stretch it or make it shorter, that deep feelanalysis within.
    Love and peace on all

No, the important thing to to do is to make time in one's life to do it.
